MASK WEARING POLICY FOR PATIENTS

Lakeside Family Practice’s Policy re Mask Wearing for Patients

For patients attending at Lakeside Family Practice with one or more of the following symptoms:

• Fever
• Coughs/colds/sore throats
• Flu like illness
• Diarrhea/vomiting
• Other contagious diseases such as Chickenpox, Measles or Whooping Cough

you and your child may be asked to wear a mask to reduce the possibility of transmission, particularly when there is increased risk in the local community.  If you or your child are unable to wear a mask we may ask you to wait in an unoccupied room/waiting area or to return to your car.
